Online survey to help identify training gaps and opportunities for strengthening knowledge of Age-Friendly Health Systems and the 4M’s in Maine

Attention Maine providers and aging network professionals

The AgingME Geriatrics Workforce Enhancement Program (GWEP) team at the University of New England and The University of Maine invites you to take part in an online survey to help identify training gaps and opportunities for strengthening knowledge of Age-Friendly Health Systems and the 4M’s in Maine. The purpose of AgingME is to improve the health and well-being of Maine’s older adults through training enhancements and practice transformation processes at the primary care practice level. Findings from this survey will be used to inform future trainings and activities carried out by the AgingME GWEP.

 We would greatly appreciate your participation. The survey will be crucial in helping to develop future GWEP workshops that include important and needed areas of training for healthcare providers, older adults, and their care partners. Please complete by June 1.
