National Survey of Individuals 60+ Living Independently
You are invited to participate in a research study to help us improve nursing home and assisted living care!
The Designation of Excellence (DoE) in Person-Centered Long-Term Care project team is looking for your input. The University of Maine Center on Aging and its research partner, The Cedars of Portland, Maine are looking for older persons aged 60 or over currently living independently (not in a nursing home or assisted living community). If you or someone you know fits that description, please take this survey about your expectations and preferences about person-centered care should you or a loved one ever need to live in a long-term care setting.
A better understanding of your concerns and integrating your preferences and priorities into the DoE will ensure its relevance for future users and for long-term care providers engaged in strategic planning.
The survey is entirely anonymous and should take you about 15-20 minutes. When you click on the link below, you will first review a project flier and an Informed Consent document that outlines how the project team will use the data it collects from survey respondents.
