Opportunity for Two Maine Primary Care Practices to Participate in AgingME GWEP
Who we are:
AgingME is a 5-year HRSA funded grant, within the Department of Primary Care at the University of New England College of Osteopathic Medicine. We are one of 48 organizations funded nationally to focus on improving health outcomes for older adults by creating a more age-friendly health system. Our partners, include the University of Maine Center on Aging, Northern Light, Maine Health, all 5 Area Agencies on Aging, primary care practices and older adults and care partners.
What we do:
We work with primary care practice (PCP) teams throughout Maine to provide age-friendly health care using the 4-Ms framework (What Matters Most, Mobility, Mentation and Medication); earn IHI Age-Friendly designation; track patient progress; access monthly ECHOTM trainings that focus on geriatrics education and participate in national conversations. Now in our 4th year of funding, AgingME is recruiting two more practice sites with a commitment to Age-Friendly care of older people. We are especially interested in recruiting rural practice sites.
What we are able to offer you, your PCP practice & patients:
- Funding for a part-time Site Coordinator to help to implement the Age-Friendly “4Ms” framework
- Staff Onboarding
- Evaluation support (University of Maine Evaluation Team)
- Opportunity to achieve IHI Age-Friendly Health Systems recognition (Level 1 & Level 2)
- Level 1: Age-Friendly Health Systems Participants, As of June 2022, 2837 Hospitals and Health Care Practices have been recognized by IHI as Age-Friendly Health Systems Participants.
- 254 GWEP primary care partner sites
- Level 2: Age-Friendly Health Systems Committed to Care Excellence, As of June 2022,
798 Hospitals and Health Care Practices have been recognized by IHI as Age-Friendly Health Systems- Committed to Care Excellence- 125 GWEP primary care partner sites
- Collaborative learning community/resource sharing with 48 nationally HRSA funded GWEPs all working on similar projects to create Age-Friendly systems
- Opportunity to provide feedback to HRSA and CMS about what matters most to your patients and your practice
- Level 1: Age-Friendly Health Systems Participants, As of June 2022, 2837 Hospitals and Health Care Practices have been recognized by IHI as Age-Friendly Health Systems Participants.
